As the parent nonprofit for the Community Loan Center of Arizona, I authorize Housing Solutions of Northern Arizona, Inc. to charge the credit or debit card designated by me, the borrower, for repayment of amounts owed to their Community Loan Center of Arizona program, documented in my loan agreement.
I certify that:
I am an authorized user of the credit or debit card. I will provide this information to a representative of the Community Loan Center of Arizona and/or Housing Solutions of Northern Arizona as payment toward any outstanding balance I owe.
I authorize the amount specified below to be charged to my credit or debit card.
I will not dispute the payment with my card issuer, provided the transaction corresponds with this authorization.
I acknowledge that this authorization is valid for the purpose described above and will remain in effect until the charge has been processed or until I provide written notice of revocation.
